 Purpose of Form:Students registered in a training program under the Training and Testing Regulation of the Private Security and Investigative Services Act, 2005 must complete this form. This allows the training entity, the ministry, and the test delivery provider Serco, to share information about a student's eligibility to write the test and apply for a licence. Training entities should distribute this form to their students and provide instructions on how to complete and submit it.
