This form is used to make an appeal to the Local Planning Appeal Tribunal (LPAT) that, according to the act/legislation, is to be filed with the municipality or approval authority. The form is to be completed by the appellant or their representative. Please use the A2 form for appeals that, according to the act/legislation, are to be filed directly with the LPAT.

This form is used to make an appeal to the Local Planning Appeal Tribunal (LPAT) that, according to the act/legislation, is to be filed directly with the LPAT. This form is to be completed by the appellant or their representative. Please use the A1 form for appeals that, according to the act/legislation, are to be filed with the municipality or approval authority.

Municipal/Approval Authority Submission(004-3050E)
This form is used by the municipality/approval authority when preparing an appeal referral package for appeals that, according to the act/legislation, are to be filed with the municipality or approval authority instead of directly with the Local Planning Appeal Tribunal. This form should not be used by appellants or their representatives.
