I attest that the information I have supplied on this form is true and correct to the best of my knowledge. I understand it is an offence to supply false or inaccurate information in the process of an application for an EDL under section 14(1) of the Photo Card Act, 2008.

I understand that if I answer, “Yes” to any of the above questions, or should my status change and my answer become“Yes” to any of the above questions, my EDL may be refused or cancelled.

I undertake to report any changes in the information given above to the Ministry of Transportation without delay.
