	Purpose of Form:	To revive a business corporation that was cancelled for failure to file a Special Notice/Annual Return or for cause (e.g. NSF cheque). At the time of revival, all outstanding Special Notice/Annual Return filings for the period of 1992 to 1995 must be attached together with the outstanding fees ($50.00 for each year not paid). Business corporations cancelled by the Ministry of Finance may also be revived.
